1. Acceptance of Terms
By accessing or using any feature, tool, or resource provided by PayerIDLookup.com ("the Platform," "we," "us"), you ("the User," "you") confirm that you have read, understood, and agree to be bound by these Terms of Service. If you do not agree with any section of this agreement, you must discontinue use immediately.
These Terms apply to all visitors, authorized users, billing administrators, healthcare practitioners, and any party accessing the Platform's services, including but not limited to Payer ID lookup, Provider NPI verification, Denial Code reference, and Claims Mailing Address Optimization tools.
2. Zero-Storage Policy for Protected Health Information (PHI)
PayerIDLookup.com maintains a strict Zero-Storage Policy regarding Protected Health Information as defined under the Health Insurance Portability and Accountability Act of 1996 (HIPAA), 45 CFR Parts 160 and 164. Specifically:
- No PHI Retention: The Platform does not store, archive, cache to persistent disk, or retain any patient-identifiable health information at any point during or after a search transaction.
- Data Conduit Architecture: The Platform operates exclusively as a pass-through data conduit. Search queries are forwarded in real time to upstream government and industry registries (such as the CMS NPPES Registry and the Stedi Payer Network). Responses are streamed back to the requesting browser session and are never written to server-side logs, databases, or file systems.
- Session Isolation: Each user session is fully isolated. No cross-session data persistence, user tracking, or behavioral profiling occurs.
3. Authorized Use and HIPAA Compliance Obligations
By using this Platform to access National Provider Identifier (NPI) records, Payer ID directories, or claims routing information, you represent and warrant that:
- You are an authorized healthcare professional, credentialed billing administrator, revenue cycle analyst, or compliance officer acting within the scope of legitimate healthcare operations as defined under 45 CFR § 164.501.
- You will use NPI numbers, taxonomy codes, and payer identifiers solely for lawful purposes including, but not limited to, claims submission, eligibility verification, payment processing, and provider credentialing.
- You will not use information obtained through this Platform to conduct unsolicited marketing, identity fraud, social engineering, or any activity that violates federal or state law.
- If you are a Covered Entity or Business Associate under HIPAA, you acknowledge that the Platform does not act as your Business Associate and does not enter into Business Associate Agreements (BAAs), as it neither creates, receives, maintains, nor transmits PHI on your behalf.
4. "As-Is" Financial Data and Insurance Eligibility Disclaimer
⚠️ Important Financial Data Disclaimer
All Payer ID codes, NPI records, claims addresses, denial code definitions, and transaction support indicators provided by this Platform are delivered on a strictly "AS-IS" and "AS-AVAILABLE" basis without warranties of any kind, whether express, implied, or statutory.
PayerIDLookup.com does not guarantee the accuracy, completeness, or timeliness of any data returned by upstream registries. Insurance eligibility responses, payer transaction support flags, and claims mailing addresses may change without notice at the discretion of the issuing carrier or government agency. Users are solely responsible for independently verifying all data before relying upon it for claims submission, payment posting, or patient billing decisions.
Under no circumstances shall PayerIDLookup.com be held liable for any denied claims, lost revenue, timely filing failures, or financial losses arising from the use of or reliance upon data obtained through this Platform.
